Systematic reviews from 2021 to 2025 conclude that tranexamic acid reduces blood loss, bruising and swelling in plastic surgery without a significant increase in clotting events.
Tranexamic acid is an antifibrinolytic drug that stabilizes clots. Common in orthopedics and cardiac surgery, its use has spread to plastic surgery.
A systematic review and meta-analysis in aesthetic surgery (2021) and another on plastic surgery in general (2024) found less blood loss, less bruising and swelling and less anemia, with no significant increase in thromboembolic complications. In facial aesthetic surgery, a 2025 review describes it as an effective adjunct that reduces intraoperative bleeding, operating time and postoperative sequelae.
Dose, route (intravenous, topical or infiltrated) and contraindications must be decided by the surgeon and the anesthesiologist for each patient.
It is not something you should demand, but it is a legitimate question at the consultation: what measures does the team use to reduce bleeding and bruising, and who is the anesthesiologist applying them.
