A prospective study of 68 women with arm lipedema found that circumferential liposuction significantly and durably improved shoulder mobility and upper-limb function, while also reducing pain, heaviness, and arm circumference.
Lipedema is a chronic adipose tissue disorder that can affect the arms, especially in advanced stages, causing pain, heaviness, and functional limitations. Although liposuction is used to reduce volume and relieve symptoms, its effect on objective arm function, particularly shoulder mobility, had not been systematically evaluated until now.
In this prospective observational study, 68 female patients with upper-extremity lipedema underwent circumferential arm liposuction. Active shoulder range of motion (abduction, flexion, and external rotation) was measured before surgery and at 1, 3, 6, and 12 months afterward, along with upper-limb disability (QuickDASH), pain, perceived heaviness, and arm circumference. Active shoulder abduction improved from 132.4° to 154.7° at 6 months and remained stable at 12 months (p < 0.001). Patients with Stage II and III lipedema showed greater absolute functional gains, reaching mobility values similar to those of earlier stages. QuickDASH scores, pain, and heaviness also decreased significantly, and arm circumference reduction correlated moderately with improvement in shoulder abduction.
The authors conclude that upper-extremity liposuction for lipedema is associated with significant and durable improvements in shoulder mobility and arm function. Beyond symptom relief and contour improvement, surgical debulking appears to restore functional movement, supporting the concept of upper-extremity lipedema as a functional disorder. Objective assessment of shoulder mobility should be considered an important outcome measure in the surgical evaluation of lipedema.
